Aloha,

My tacrolimus levels were pretty consistent for 6 months but then started dropping and they have upped my tac over the past few months from a daily total of 3.5 mgs to now a total of 5 mgs. I can't think of any changes in my diet and I always take my meds with food. Maybe I should take them without food and see what my levels do.....they tell me not to worry about it and they don't know why the tac levels are going down. I am not worried but it is in the back of my mind. I am doing fantastic, I paddle on an outrigger canoe this morning! Any feedback would be appreciated.

Awesome news, Starla.

Here's my take on Prograf; you need to be consistent, both about when and how you take it. BUT, they told us at Duke to take it one hour before or two hours after eating, so you would need less of it in your body to acheive the same levels. The food DOES absorb some. Of course in our now thankfully busy lives, it's not always possible to acheive, but it's what I strive for.

FYI- I take 2.5 mg 4 days a week, and 3 mg 3 days a week to maintain a level of around 7. You may need a higher level since you're 'fresher'. I always try to get blood tests on days after I take only 2.5.

i wouldn't worry too much - that's why they take our blood test every 6 weeks or so - levels do fluctuate. I am sure your Doc's would let you know if there is a concern.

My Prograf and Cellcept say they are to be taken on an empty stomach and for the most part I do that as best I can but am not locked into it.

Tom- I think that's why most hospitals try to get you on a 10:30-10:30 schedule. It's the easiest times of day to try and have an empty stomach. Like you, I take my Prograf and Cellcept together at those times. Hope you are feeling great!

Ehm I don't know but my stomach is never empty at 10:30. It seriously is never empty because I get nauseous if there's nothing in my stomach. So I always take it at 11 and 11.

One other thing, my doc wants to try to let me take my med once a day instead of twice. Does Anybody have an experience with this? My doc says that my levels would be more constant if I take prog. Once a day.

Well i must be the odd man out here. While in the hospital they would give me my morning meds around 8:30 and the same at night.

Upon getting home I found that those times didn't wok for me -not a morning person - so I changed to 10:00 and 10:00 - gives me time to get bloodwork done without being there at the the crack of dawn. Did I say I wasn't a morning person???

I rather take less med so for me I take my first dose at 6:30 AM and then again at 6:30 PM.

Why so early? Because I need to eat frequent meals and I can start my first meal at 7:30 Am and then end at 4:30 PM. Wait two hours and take my Prograf at 6:30 and then eat dinner after seven. Then I can have some light snacks after dinner.

I am trying to gain some weight and with a high protein low carb, moderately high healthy fat diet I need to eat meals and snacks often.

Once I get to my ideal weight, I will probably cut back every now and then.

So far so good!
