I am not sure which is better for the kidney, but I do know that the two drugs are related. Prilosec consists of a mixture of two mirror-imaged compounds whereas Nexium is just one of these mirror compounds. I hope that your doctor can help you find the one that will work the best for you and your kidney. Best of wishes.

Pantoprazole can also be used. I would check to see if your transplant team has any experience with these and if they have no strong medical reasons for selecting one over the other then I'd pick the one that costs less on a per day basis. I stopped using these as soon as I could and I recommend the same to you (I have no significant reflux issues and only used them after the transplant until I convinced my docs to let me stop). I used both omeprazole and pantoprazole. My wife has a continuing reflux problem and I got her to switch from omeprazole to pantoprazole to save a few bucks. None of these has any evidence of damaging the kidneys.
