I was diagnosed with diabetes few months ago and take insulin. I take tacrolimus, myfortic and steriod. How did you manage diabetes caused by tacrolimus - reducing it or switching to another immuno drug?

Hi Bala. I'm not sure whether changing the drug will help or not. Because the steroid can also cause diabetes. And if these drugs work for your graft, you don't want to play with them too much. A really good place to learn about diabetes is Diabetes Daily (dot com). I personally wouldn't go too low carb, due to wanting to baby the kidney, but you can get some good help and support there. I would also get the books "Think Like a Pancreas" and "Using Insulin". Sorry, I can't help as far as getting rid of the diabetes.

thanks cora for the links, neph reduced the tacro by a bit and stopped the night insulin.

Hi Bala, Blame the Tacro! 20% of patient using it become Type II diabetics soon after starting. Combined with Prednisone the outcome is wildly erratic blood sugars. Perhaps this is why doctors try to lower the doses of them to aid control. As for switching medications, every medication has it's faults. The survival of the Tx is always the top priority, All decisions are weighed against it. You can understand why switching meds is a second priority, as Diabetes is manageable. As, ask, ask your Tx team about other options. Otherwise consider it as a party favor and the price of the ticket to survive. Stay well. |

How about trying to dump the steroid. I'm 3 years out and have been on a steroid free regimen for the whole time. My meds now are 3mg Prograf daily and 3mg Rapamune daily. I follow the Univ. of Pittsburgh protocol, which is steroid free,l and have had no problems with the meds I'm a 1/6 match, also with PKD. I've still got my native kidneys.

Yes, I can read your post aleseinFL Bala, like Gene, I am a kidney transplant patient, and have never taken steroids since leaving the hospital. Seattle Swedish and some others no longer keep their patients on prednisone. I wonder if other transplant centers would be willing to make the change, at least to avoid diabetes? Does anyone know? Can one just at least slowly go off of it?

I was diagnosed with diabetes after being on the Prednisone & Prograf combo for 6 mos. post transplant. I am 14 years post transplant in April 2015. I was switched to Rapamune and 2.5mg of Prednisone but it was too late. The best way to deal with diabetes is to stop drinking regular sodas and sweet drinks, etc. check blood sugar three times daily. |
